Stem Cell Physical Therapy: The Rehab Plan That Protects Your Injection.
You’ve done the hard part. You researched it, you paid for it, you had it done. What happens in the next six months decides whether it works.
We build the physical therapy around the procedure you already had — written week by week, so you always know what you’re cleared to do, what waits, and what stops.

Ron was headed for a double knee replacement. Two years later he played 18 holes three days running.
You had the injection. Now what?
The Injection Takes An Hour. The Recovery Takes Six Months.
Here’s what usually happens next. You leave the clinic with a printed sheet and two words: “take it easy.” For how long? Easy compared to what? Nobody says.
So you do what most people do. You rest too long and wonder why nothing’s changing. Or you feel good at week three, go back to the gym, and spend the next month nursing a flare-up. Or you find a physical therapist — a good one, even — who has never worked with someone after a stem cell injection.
Amy messaged me recently. Two weeks after her injections, on her very first session back, her PT had her pushing a 200-pound sled. She asked me if that was normal.
It isn’t. But I don’t blame the therapist. Most PTs never see enough post-injection clients to know what those first weeks should look like. That’s the gap we fill. — Dr. Chris Garcia
The definition
What Is Stem Cell Physical Therapy?
Stem cell physical therapy is a rehab program built specifically for the months after a stem cell injection. Outside the US it’s often called stem cell physio or physiotherapy; some clinics call it stem cell rehab or rehabilitation. The name changes. The job doesn’t.
The cells respond to what you ask the joint to do. Ask too little and nothing remodels. Ask too much too soon and you flare it. Stem cell physical therapy is the plan that gets that dose right — week by week, for your joint, your procedure and your goal.
It isn’t the same as regular physical therapy. Regular PT works to a standard timeline for an injury or surgery. After an injection, the timeline has to follow what the cells are doing — slower in some places, and faster than you’d expect in others.
Why ordinary rehab gets it wrong
Three Ways People Waste A Good Injection
Every one of these is avoidable. You just need a plan written by people who do this every day.
Too much, too soon.
You feel better at week three and assume you’re healed. You’re not — you’re in the window where the joint is most easily set back. A flare-up here can cost you weeks.
Too little, for too long.
Resting “just to be safe” feels responsible. But tissue remodels along the lines of the stress you put through it. Rest tells it nothing, and the muscle you lose in the first eight weeks is the muscle that protects the joint for the next twenty years.
Guessing.
A generic protocol, a YouTube video, a friend’s program. None of them were written for your joint, your procedure or the sport you actually want back.
How it works
Six Phases, In Order. Twenty-Four Weeks.
Every program follows Dr. Chris’s Recovery Method. The dates shift with your joint, your procedure and your goal. The order never does.
Active rest
Walking only. Protect the site while it does the work you paid for.
Mobility
Range and flexibility first. Nothing loaded, nothing forced.
Activation
Waking the muscles around the joint. Bands enter here.
Reintegration
Back into a real fitness routine, built for volume before load.
Strength
Weight training returns. First running, if running is the goal.
Return
Full routine, back to normal, back to what you love.
Every phase opens that phase’s articles and videos in the Recovery Library. At week 24 you sit down with your six-month review and decide what comes next.

The clearance system
Every Week, You Know Exactly What You’re Cleared To Do.
Clients tell us they’d pay for this part on its own. Nothing is left to interpretation — every movement sits in one of three lanes, and lanes change as you earn them, not when the calendar says so.
Getting moving again
Safe from the first week. Gets the joint moving again without stressing the injection.
- Gentle movements to loosen the joint
- Slow, relaxed breathing
- Stretching guided by your therapist
Building strength
Added once you clear the milestones for your phase.
- Light weights and resistance bands
- Step-ups, stairs and balance work
- Walking, biking or swimming at an easy pace
Back to what you love
Held back until the final phase. This is where people who go it alone get hurt.
- Heavier lifting
- Jogging, longer walks and hikes
- Golf, tennis and pickleball

“I actually did not utilize any kind of stem cell physical therapy experts after my first injections, and I didn’t have a whole lot of results… In hindsight, I did way too much way too soon… The second time, about two and a half months in, I was just walking up the stairs at work and all of a sudden I went — that doesn’t hurt.”
Sue, 59Knees · virtual, from Minnesota · second round of injections“We’re in a small community. We have limited physiotherapy resources, let alone anyone who is an expert with stem cell recovery… I feel like I had way less flare-ups, much better guided care… I healed faster when I followed my plan.”

Doctor of Physical Therapy from Washington University in St. Louis, then an elite sports residency at The Ohio State University, advanced training with Dr. Shirley Sahrmann, and three years as a physical therapist at the U.S. Olympic Training Center.
He has been rehabilitating people after regenerative procedures since 2008 — starting with cartilage transplants, before anyone was saying “stem cell” out loud. Olympic athletes, UFC, professional tennis, NFL Combine training. All of them came back from a procedure asking the same question you’re asking now.
He owns Sports Performance Physical Therapy, teaches as adjunct faculty at San Diego State, hosts The Stem Cell Recovery Podcast, and built the method these programs follow.
Every client goes through Dr. Chris’s Recovery Method. Six phases across twenty-four weeks, with a clearance rule at every stage for what you can do now, what waits, and what stops. He built it, and he trained every therapist who runs it — Dr. Shane, Dr. Brandon, Dr. Jordan and Dr. Pete, all four of them Doctors of Physical Therapy. They write your plan against the method with him, and he reviews how your joint is responding as you move through it. Read the full story →

Stem Cell Physical Therapy: What People Ask Us
Do I need physical therapy after a stem cell injection?
The injection starts the repair. What you do with the joint over the next six months shapes it. Without a plan, most people either rest too long or load too early — and both cost you results.
When should I start physical therapy after stem cell therapy?
The program starts the day of your injection. Weeks 1–2 are active rest, with walking only. Mobility work begins around week 2. The exact timing depends on your joint and your procedure, and your plan spells it out.
Can you exercise after stem cell injections?
Yes, but in order. Walking comes first, then mobility, then bands, then a full routine. Weights usually return around week 16, and running, if it’s your goal, in the same window. Going early is the most common way people set themselves back.
What should I not do after stem cell treatment?
Don’t test the joint because it feels good, don’t jump back into your old routine, and don’t follow a program written for surgery. Your plan lists what’s off-limits each week, so you never have to guess.
Is it normal to have more pain after a stem cell injection?
Soreness in the first days after an injection is common. Pain that keeps climbing, or comes with swelling, heat or fever, needs a call to the doctor who did your injection. During the program, we adjust your loading if a flare-up shows up.
How long is stem cell recovery?
Our programs run 24 weeks across six phases. Most clients feel better well before that. The last phases rebuild the strength and power that keep the result.
Is stem cell physio the same as stem cell physical therapy?
Yes. Physio and physiotherapy are the terms used in the UK, Canada, Australia and much of the world. We work with clients in all of them through the virtual program.
Do you perform the injections?
No. Wherever you had yours — down the street or in another country — we handle everything that happens after it.
Do you take insurance?
We do not bill insurance for you. We’ll go over cost and options on your call.
